Health Ministries Free Sports Physicals for Halstead Students
The Health Ministries Clinic in Halstead will be offering 24 free sports physicals for students in grades 7–12 (2025-26) on Thursday, May 15, from 8:00 AM to 12:00 PM.

Time slots are limited—please reserve your appointment using the link provided.

Be sure to read all additional information before arriving for your scheduled time.
Paperwork (Sports Physical Packets can be picked up in the HHS office)
All paperwork must be dated May 1st or later to comply with KSHSAA regulations.
All paperwork will need parental/guardian signatures in the appropriate places or will be considered
invalid and a sports physical cannot be completed.
Please collect KSHSAA and parental consents ahead of the sports physical event and as best as
possible, have in order of the sign up.
HMC staff will take all paperwork and forms upon arrival for the check-in area to look over and retain
until students present to this area.
Students will get checked in according to their time slot and then be given their paperwork

nurse’s station.
Parental Consent Requirements
Students 12 and younger will be REQUIRED to have an adult/parent/guardian present at the time of
their sign-up time, and during the sports physical.
Students aged 13 years and older must meet one of the following conditions:
o Parent/guardian is present during the sports physical OR
o Student provides a signed HMC parental consent form (available in English and Spanish)
allowing providers to complete the sports physical without parental presence.

Regional Baseball @ Sylvan-Lucas
Halstead will be the 5 seed and will travel to Sylvan-Lucas on Wednesday, May 14th for its quarterfinal game. The game begins at 5:00 pm.
Congratulations to the HHS Golf Team on their 6th place finish at the Marion County 36 hole tournament. Individually Jacob Rose finished 6th at both Marion and Hillsboro and Maxx Linton finished 10th at Hillsboro.
